
Every province in Canada pays for semaglutide the same way in one respect: as a diabetes drug, for people who meet a diabetes criterion, through a prior-authorization step the prescriber has to complete. None of them funds it for weight management on its own. What differs — and what decides whether your claim goes through at the counter — is the machinery: what the request is called, who files it, which dose is capped, and which programme you have to be enrolled in before any of it applies.

The rule that applies everywhere
Public coverage follows the Health Canada-approved indication. Semaglutide is approved for type 2 diabetes under one brand and for chronic weight management under another, at a higher dose — and it is the weight-management product that no provincial plan funds. That is why a prescription written for weight is a private-pay prescription, and why asking "is Ozempic covered" gets a different answer depending on why you are taking it.
Prior authorization is a prescriber task, not a patient task
Special Authority in BC, Limited Use codes and the Exceptional Access Program in Ontario, Special Authorization in Alberta, exception-drug authorization in Quebec — different names for the same idea, and in every case the paperwork is filed by the prescriber. The most common reason a Canadian patient is quoted an unexpected full price at the pharmacy is that this step was not completed, not that the drug is uncovered. If you are told it is not covered, ask specifically whether the authorization was submitted and what the answer was.
Private plans are where weight-management claims actually get paid
Workplace insurance is the realistic route for anyone without diabetes. The wording varies plan by plan: some cover the weight-management product under an obesity benefit, some exclude weight-loss drugs by name, and some cover it with an annual maximum that runs out mid-year. Ask the insurer three questions in writing before you start — is the indication covered, is a special authorization form required, and is there a maximum. Verbal answers from a call centre are not worth much when a claim is denied eight months later.
If nothing covers it
Then the decision is a budget decision about an ongoing cost, not a one-off.
