Quebec's system is different from the rest of Canada
Quebec is the only province where prescription drug insurance is mandatory. If you have access to a private group plan through work, a professional order or a union, you must take it; if you do not, you must be registered with the RAMQ public plan. This matters for weight medication because which plan you fall under decides everything else, and many people who assume they are on RAMQ are actually required to claim through a spouse's group plan first.
Exception drug, not general benefit
Semaglutide is handled under Quebec's exception drug measure. The Minister of Health, on advice from INESSS, sets recognized indications for payment, and the drug is reimbursed when the prescription matches one of them. The prescriber files an authorization request; approval is what unlocks payment. There is also a separate exception patient measure for indications outside the recognized list, which is a case-by-case route rather than a general one.
The practical consequence is the same as in the other provinces: the recognized indications are diabetes indications. Weight management on its own is not what the public plan is paying for, and the authorization is tied to the indication written on the request.
What RAMQ registrants pay even when a drug is covered
The public plan is not free at the counter. Registrants pay a monthly deductible and coinsurance up to a monthly maximum, with the amounts set annually, and an annual premium collected through the income tax return. Seniors receiving the guaranteed income supplement and people on last-resort financial assistance pay less or nothing. Budget for the contribution rather than assuming coverage means no cost.
Private plans in Quebec
Group plans in Quebec must be at least as generous as the public plan for the drugs on the RAMQ list, but they can be more generous — and weight-management coverage, where it exists, comes from that "more generous" part. Read the booklet, not the summary.
